How Does an Embryo Change in Just 5 Days? From Fertilization to

Blastocyst

When you begin IVF treatment, one of the most exciting and emotional parts often starts

after egg retrieval.

The eggs have been collected, fertilization has taken place… But what happens next?

One of the questions I hear most often from my patients is:

“Doctor, what is my embryo doing right now?”

The truth is that an embryo goes through an incredible transformation in just a few days.

What begins as a single cell gradually becomes a much more organized structure, preparing

for the next important step: implantation.

So, let me walk you through the embryo’s journey, day by day, just as I explain it to my

patients in the clinic.

Day 1: It All Begins With One Cell

Everything starts when the egg and sperm come together.

During IVF treatment, the egg and sperm are brought together in the laboratory. When

necessary, ICSI (Intracytoplasmic Sperm Injection) may be used, where a single sperm is

injected directly into the egg.

After successful fertilization, we can typically see two pronuclei, known as 2PN, representing

the genetic material from the mother and father.

This is the very beginning of the embryo’s journey.

It is incredibly small — but there is already a remarkable process taking place.

Day 2: The First Cell Divisions

Around 48 hours after fertilization, the embryo usually reaches the 2–4 cell stage.

The embryo has started dividing.

At this point, our embryologists carefully monitor not only the number of cells but also how

those cells look and how evenly the embryo is developing.

And this is something I always remind my patients:

Embryos do not work like a clock.

Not every embryo develops at exactly the same speed. A few hours of difference does not

automatically mean that an embryo is not developing well.

What matters is the overall development pattern.Day 3: The Embryo Continues to Grow

Around 72 hours after fertilization, the embryo commonly reaches the 6–8 cell stage.

This is often referred to as the cleavage stage.

At this point, we look at several characteristics of the embryo, including:

Number of cells

Cell symmetry

Fragmentation

Overall appearance

Developmental pattern

So when a patient asks me:

“Is an 8-cell embryo definitely a good embryo?”

I usually explain that it is not quite that simple.

The number of cells is important, but it is only one part of the picture.

We want to understand how the embryo is developing, not simply count the cells.

Day 4: The Morula Stage

By around Day 4, the embryo begins to develop into a structure called a morula.

The cells continue to multiply and become more tightly connected to one another.

The embryo now looks very different from the individual cells we saw during the earlier

stages.

It is becoming a more organized structure.

And the next major milestone is just around the corner:

The blastocyst.

Day 5: Blastocyst – A Major Milestone

This is one of the stages my patients are most curious about.

Around Day 5 after fertilization, some embryos that continue to develop successfully reach

the blastocyst stage.

A blastocyst is a much more developed and organized embryo.

At this stage, we can identify different groups of cells that will have different roles in a future

pregnancy.

So, when we reach Day 5, the embryo is remarkably different from the single cell we started

with only a few days earlier.Day 3 or Day 5 Transfer – Which Is Better?

This is another question I hear very often:

“Doctor, is a Day 5 embryo always better?”

My answer is:

Not necessarily.

Every patient is different.

In some situations, a Day 3 embryo transfer may be appropriate. In other cases, it may be

preferable to continue embryo culture until Day 5 and transfer a developing blastocyst.

The decision depends on many factors, including:

The number of embryos available

Embryo development

Maternal age

Previous IVF treatments

Embryo quality

The patient’s individual medical history

The condition of the uterus

So I would not recommend following general statements such as “Day 5 is always better”

or “Day 3 is more successful.

There is no single IVF plan that is right for everyone. The best treatment plan is the

one tailored to the individual patient.

What Do 4AA, 5AA and 3BB Mean?

When you receive your embryology report, you may see numbers and letters such as:

4AA, 5AA, 4AB or 3BB.

At first, they can look like a secret code!

In fact, these are morphological grading systems used to describe the appearance and

development of a blastocyst.

But there is something very important I want my patients to remember:

A beautiful-looking embryo does not guarantee pregnancy.

Likewise, an embryo with a lower morphological grade does not necessarily mean that

pregnancy is impossible.

The way an embryo looks under the microscope and its genetic status are not the same

thing.Does a Beautiful Embryo Mean It Is Genetically Normal?

No.

This is an important point.

An embryo may look excellent under the microscope, but morphology alone cannot tell us

with certainty whether the embryo has a normal number of chromosomes.

When medically appropriate, Preimplantation Genetic Testing (PGT) may be considered.

This involves taking a small biopsy from the embryo and analyzing its genetic material.

However, PGT is not automatically necessary for every patient.

Whether genetic testing is appropriate should be evaluated individually, considering factors

such as age, medical history and previous reproductive history.

If Your Embryo Does Not Reach Day 5, It Is Not Your Fault

There is another subject I particularly want to address.

Sometimes patients become very upset when they hear that an embryo has stopped

developing before reaching the blastocyst stage.

They may ask:

“Did I do something wrong?”

“Was it something I ate?”

“Did I not take good enough care of myself?”

Please do not blame yourself.

Embryo development is a highly complex biological process influenced by many factors,

including egg quality, sperm quality, genetic factors, maternal age and other biological

characteristics.

An embryo not reaching the blastocyst stage does not mean that you did something

wrong.

Our role is to carefully monitor embryo development and make the best possible decisions

based on the embryos available to us.

The Embryo’s Story Is More Than Just Five Days

From fertilization to blastocyst, only a few days may pass.

And eventually, a developing embryo may reach the stage at which it can be considered for

transfer into the uterus.

I often tell my patients:

“Don’t think of your embryo as just a picture. Think of it as a story of development.

Our job is to carefully follow that story at every stage, understand how the embryo is

developing, and determine the most appropriate transfer strategy for you.

Because in IVF, every embryo is different, every patient is different, and every treatment

journey is unique.

Sometimes, the most important thing is not simply how an embryo looks, but how it

develops.
